We believe healthcare starts with listening — not a 10 minute visit and a script, or as Shelby says “bandaids aren’t treatment plans”. As your Direct Primary Care provider, we spend the time it takes to understand what is really going on, so we can treat the cause, not just the symptom.

The WHY

$80/month

One-on-one unlimited visits with the same provider, every time. Chronic & complex disease $ medication management, controlled substance monitoring, preventative care, sick visits, unlimited messaging. 1-3 hour message response between hours of 7A-10P.

The BASICS

$60/month

Includes 2 regular visits & 1 sick visit/year. Lab interpretation, imaging when needed, simple disease and medication manaement, preventative care. Direct messaging still available with 1-2 business day response.

Women’s Health

$20/month

Contraception counseling, acute & chronic women’s health concerns — excludes hormone management. Does not include any primary care concerns or chronic conditions not related to women’s health.

Yearly Well-Woman

$40/year ADD ON

Includes well-woman pelvic exam, PAP testing, mammogram order (age appropriate or at provider discretion), women’s health counseling, genetic risk calculation and genetic testing when applicable.

Functional Medicine Initial

$250 one-time fee

Includes in-depth lifestyle, diet, sleep, body system, mental health, discussion & counseling for root-cause identification. Functional lab panel ordering and interpretation by provider. Labs may be billed to insurance or provider negotiated cash pay price (patient responsibility). Detailed treatment plan including diet & nutition plan, tailored exercise/movement plan, supplement regimen, and direct messaging to provider for accountability or questions.

** Initial Styku body composition scan for determining starting point and tailoring patient goals and treatment plan. No additional cost to patient.

Functional Medicine Memberhip

$120/month

Ongoing lifestyle, diet, sleep, body system, mental health, discussion & counseling for root-cause treatment. Functional lab panel ordering and interpretation by provider for follow up & progress tracking. Labs may be billed to insurance or provider negotiated cash pay price (patient responsibility). Detailed treatment plan changes when needed including diet & nutition plan, tailored exercise/movement plan, supplement regimen, and direct messaging to provider for accountability or questions.

**Will include Styku body scan every 3-6 months for progress tracking. No additional cost to patient.

Hormone Optimization

$200 Initial One-Time Fee / $75/month Continuation

**Serving both Men & Women

Includes initial one-on-one visit for diet, lifestyle, sleep, mental health/mood, sexual function symptom evaluation & exam. Lab ordering & provider interpretation. HRT treatment plan using bio-identical compounded medications, tailored supplement regimen, diet & nutrition plan, exercise/movement plan.

Continuation includes all of the above, ongoing monitoring of symptoms and lab levels. Changes & dose adjustments at provider discretion and patient discussion. One-on-one visits when needed, therapeutic blood donation when needed (TRT).

**Includes Styku body composition scan for initial eval & ongoing progress tracking. No additional patient cost.

Child Add-On

$30/month with Parent Membership

Add your kid/kids to your membership for personal, convenient care for the whole family, from well-child checkups to sick visits. We love the littles & work delicately to help them have a great, fun experience. Fully support parent informed decisions regarding health of child. Supportive of vaccination choice and provider dedicated to spending the time for education and explanation.

Community Same-Day Sick Visit

$50/visit

Not a member? Get seen the same day for colds, flu, infections, and minor illnesses. No long waits and no insurance needed.

**Telehealth is an option at provider discretion.

Try us out with a single visit at a flat rate. Includes anything you’d like to address. Focus on root-cause disease identification, personalized treatment plan, supplement regimen, medications when needed. Chronic disease management & preventative care.

This is also a good option for those who only see a provider once/year. Additional visits would be $50/visit.

Tele-health Establish Care & Medication Refill

$65 One-Time Visit Fee

Focused 30 minute virtual visit designed to get you connected with care quickly. This visit covers medication refills and basic health information gathering as we establish care for you. It is the perfect starting point if you need an immediate refill while we get to know you and your health history.
